There are practical problems in the provision of health services which mitigate against the success of the service, quite separate from the "proven" efficacy of treatments and procedures. I will highlight these issues which relate to ear health programmes in the Top End and particularly refer to a hearing programme in a remote community (pseudonym "Merrepen" ) .
